TSCN seeks to improve referral coordination by helping participating practices communicate more efficiently and by supporting cleaner transitions between providers involved in a patient’s care. Care coordinators may assist patients with scheduling, communication, document collection, and coordination between offices in order to improve continuity and reduce administrative friction.
Patients remain free to choose their doctors and participate actively in all healthcare decisions. TSCN does not require exclusive referral relationships, mandate utilization patterns, or interfere with independent medical judgment. Referral decisions remain individualized and based on clinical considerations, patient preferences, logistical considerations, and professional relationships.
