TSCN is designed as a coordination and communication infrastructure intended to support participating independent practices. The network focuses on improving organization surrounding patient intake, referral communication, follow-up coordination, documentation flow, and care continuity between participating providers and care coordinators.
Participation does not require doctors to relinquish ownership of their practices, alter their independent clinical judgment, or terminate existing hospital or professional relationships. Participating practices continue operating independently while utilizing shared coordination tools and communication systems intended to reduce fragmentation and administrative inefficiency.
As the system evolves, participating doctors may gain access to additional operational support tools, coordination workflows, communication infrastructure, educational resources, and collaborative opportunities designed to support organized independent medical practice.
