Membership

Independent medicine. Coordinated care. Informed choice.

TSCN — the Tri-State Specialized Care Network — connects independent doctors through shared Care Coordinators, referral pathways and technology. We are building a way for patients, doctors and care teams to work together while preserving the patient’s choice and the doctor’s independent clinical judgment.

For independent doctors

TSCN Doctor Membership and Services

Mission: TSCN helps independent doctors work together through coordinated care, shared technology, practical services and patient-centered referral pathways while preserving independent clinical judgment and patient choice.

Membership Always Free

Doctor membership creates no membership fee or financial obligation. Paid Guild services are optional and available only when a member chooses to use them. Participation in TSCN care pathways requires one-time credentialing and at least one unit of Care Coordination, as described in the doctor pledge. Service fees are for services, never payment for referrals.

Why private-practice doctors should join

Your practice remains yours. TSCN does not employ you, buy into your practice or direct your clinical care. Membership gives independent doctors a way to build together.

Care coordination

Help patients, appointments and necessary information move between practices.

Referral connections

Connect with trusted colleagues when their skills fit the patient’s needs. Referrals serve care; the patient chooses.

Shared technology and marketing

Participate in tools and collective visibility that individual practices may find difficult to build alone.

A voice in what we build

Contribute ideas, give feedback, invite trusted colleagues and take part in member education.

Three ways to participate

Doctor Membership Pledge

For independent doctors who want to coordinate care and build a stronger collaborative while retaining their practice and clinical independence. Membership itself is free.

What the doctor pledge supports

  • Stay independent while working with trusted colleagues.
  • Help patients and necessary information move between offices.
  • Contribute ideas, invite colleagues and participate at the level that fits your practice.

Sponsor Pledge

For organizations that want to support TSCN programs, education and infrastructure. Sponsorship provides no patient referrals, preferential routing, clinical influence or access to patient data. Amount and purpose are recorded in the pledge.

Why sponsors should participate

Help build the coordination, education and connections that patients and independent doctors need. Support a defined TSCN event or program, receive clearly disclosed recognition, and contribute useful education without influencing medical decisions.

  • Support guest speakers, learning and professional connections.
  • Receive recognition described in the Sponsor Pledge.
  • Agree on the sponsorship purpose and amount in advance.

Patient Membership Pledge

For patients who support informed choice and independent medical decisions in every setting of care. The pledge supports decisions based on evidence, the patient’s goals and the clinician’s judgment, free from inappropriate outside influence. Signing creates no fee or financial obligation.

Why patients should join

Your medical decisions should be made with your treating clinicians, guided by evidence, your needs and your goals. Join to support informed choice, clear explanations, second opinions and freedom from inappropriate financial, corporate, payer or network influence in any setting of care.

  • Keep the freedom to choose your clinician and seek another opinion.
  • Support honest explanations of options, costs and conflicts of interest.
  • Bring the patient perspective to better communication and coordination.

Patient Membership and Newsletter Registration

I believe in the value of preserving and enhancing private practice medicine.

Choose a newsletter, tell us how you would like to help, or share your thoughts. Contact permissions are optional and separate. Selecting a support option does not give permission to contact you.

Please do not include medical records, symptoms, or confidential information. This is not a medical intake form. To withdraw previous permissions, use the withdrawal option below with the same contact details; TSCN will review your request before further outreach. Withdrawal requests take priority over other contact selections. You can also contact support@tscn.care. Submissions are stored in TSCN’s Jotform account, not in the local website database. Supporter membership does not enroll a clinician as a participating medical practice.

Upcoming Events to Sponsor

Tuesday, October 27, 2026 — TSCN Education & Development Meeting

Tuesday, October 27 at 6:00 PM | The Hyatt, Hasbrouck Heights, New Jersey

The intricate process of creating the THIRD OPTION.

Guest presenter: Carlisle L. St. Martin, MD.

Sponsorship opportunities to discuss: guest speaker honorarium, dinner support and event materials. Availability, amount and recognition will be agreed with TSCN.

Additional event sponsorship opportunities will be posted when dates and details are confirmed. Contact support@tscn.care to discuss a future program.

Trust and patient choice

Clinical decisions belong to the patient and treating clinician. Patients may choose doctors inside or outside TSCN. Membership and sponsorship do not purchase referrals or influence medical recommendations.

Build with us.

Doctors: call (732) 800-5762 or email support@tscn.care to speak with a Care Coordinator about membership.

New patients: call (732) 800-5548. Existing patients: call (877) 357-7939.

Call us now. Someone will answer no matter what time you call—either our staff or our highly trained AI assistant Jessie.

Do we take your insurance? Coverage depends on the practice and service. Ask the treating office or a Care Coordinator to help you confirm; membership is not an insurance plan.
