Tri-State Specialized Care Network

TSCN (Tri-State Specialized Care Network) helps patients connect with independent doctors for spine, joint, pain, rehabilitation, chiropractic, physical therapy, and related musculoskeletal care.
Rather than simply searching a directory, patients speak with a TSCN care coordinator who helps understand their situation and identify doctors who may be an appropriate fit for their needs, location, insurance, and goals.
Patients are then provided with information about participating doctors, including links to their individual TSCN profile pages, so they can review and choose the doctor they feel is right for them.
Once a patient selects a doctor, the TSCN care coordination team can help facilitate scheduling and assist with coordination throughout the patient’s care journey.
