For Doctors & Patients
For Doctors
Independent by choice.
Stronger together.
TSCN (Tri-State Specialized Care Network) is a collaboration of independent specialists who believe there is a better way forward—one that preserves autonomy while improving care, efficiency, and the doctor experience.
You didn’t enter medicine to become an employee.
There is a third option.
The reality for independent practices today
Across the Tri-State area and beyond, independent specialists face unprecedented pressure.
The third option: TSCN
TSCN is a doctor-led network built by independent doctors, for every doctor who cares for spine and joint patients.
We coordinate without consolidating. We collaborate without compromising independence.
Three paths. Three very different outcomes.
1. Hospital Employment
You become an employee.
2. Private Equity
You become an asset.
3. TSCN – The Third Option
You remain independent.
We’ll connect you with the right person to guide you.

For Patients
Better care happens when doctors collaborate.
TSCN (Tri-State Specialized Care Network) connects independent, trusted specialists across the Tri-State area to help you receive coordinated, personalized care-guiding you at every step.
Healthcare today can feel overwhelming.
You may see multiple doctors, repeat your story, wait for answers, or feel unsure about the next step. Important information can get lost and care can feel disconnected.
A better experience is possible.
TSCN is a doctor-led network of independent practices that work together for you.
We coordinate without consolidating. We collaborate without compromising independence.
Independent. Collaborative. Patient-centered.
We’ll connect you with the right person to guide you.

Patient Membership
I believe in the value of preserving and enhancing private practice medicine.
Choose a newsletter, tell us how you would like to help, or share your thoughts. Contact permissions are optional and separate. Selecting a support option does not give permission to contact you.
Please do not include medical records, symptoms, or confidential information. This is not a medical intake form. To withdraw previous permissions, use the withdrawal option below with the same contact details; TSCN will review your request before further outreach. Withdrawal requests take priority over other contact selections. You can also contact support@tscn.care. Submissions are stored in TSCN’s Jotform account, not in the local website database. Supporter membership does not enroll a clinician as a participating medical practice.

TSCN is not an emergency service. If you believe you have a medical emergency, call 911 or seek care at the nearest emergency department.
What we treat
Spine Surgery
For complex spine conditions that haven’t resolved with conservative care. TSCN spine surgeons emphasize restraint — surgery only when truly necessary, with minimally invasive and robotic options when it is.
Physical Therapy
Physical therapy may support recovery, rehabilitation, and management of some spine and joint conditions without medication. It may be considered as an early nonoperative option when appropriate after clinical evaluation.
Chiropractic
Manual care and alignment, integrated with the rest of your medical team rather than working in isolation.
Pain Management
For chronic pain that needs more than physical therapy but doesn’t yet require surgery. Procedural and pharmacological approaches, carefully prescribed.
Joint Replacement Surgery
For knee, hip, and shoulder conditions that limit your daily life.
How it works
Call
A care coordinator listens to your situation — your pain, your history, what you’ve tried.
Triage
An independent doctor evaluates the patient and determines what care may be appropriate. Surgery is considered only when clinically indicated.
Meet
Patient Support may help connect you with a participating practice based on the general request and location, subject to practice fit and appointment availability.
Coordinate
Your providers communicate through TSCN throughout your treatment — and so do we, with you.
Even our coordinators have coordinators.
Jessie, TSCN’s AI receptionist, can receive questions and appointment requests at any time. A care-team member can review the request and follow up.

Insurance accepted
Participating independent practices may accept the following coverage, depending on the practice, location, claim, authorization, and insurer:
- Workers’ Compensation — New York and New Jersey
- No-Fault auto insurance (motor vehicle accident coverage)
- Personal injury cases (liability and litigation)
- Medicare
Participation and benefits must be confirmed for the selected independent practice.
A care coordinator can help identify participating practices that may accept your coverage, but the practice and insurer must verify network participation, claim authorization, benefits, and possible patient costs before care.
Workers’ compensation patient? Call Patient Support at 732-800-5548 before traveling.
If available, have your claim number, date of injury, employer, insurance carrier, adjuster contact information, injured body area, and any authorization details ready.
TSCN cannot guarantee claim acceptance, authorization, appointment availability, or coverage.
Common questions
Does TSCN cost extra?
TSCN does not charge patients for calling Patient Support.
Medical evaluation and treatment are billed by participating independent practices. Before care, ask the practice and your insurer or workers’ compensation carrier to confirm authorization, benefits, and any possible patient responsibility.
Will I always see the same doctor?
Not necessarily. Continuity depends on the specialty, participating practice, doctor availability, location, insurance or claim authorization, and the patient’s choices. Ask the practice which clinician will provide care and how follow-up is handled.
What if I’m not sure what kind of care I need?
Call Patient Support at 732-800-5548. A care coordinator can listen to the general concern, organize the request, and help identify a potentially relevant participating practice. An independent clinician—not a care coordinator or AI—determines what evaluation or treatment may be appropriate.
What insurance do you accept?
Participating independent practices may accept Workers’ Compensation in New York or New Jersey, No-Fault auto insurance, personal-injury matters, or Medicare, depending on the practice, location, claim, authorization, and insurer.
Participation and benefits must be confirmed for the selected independent practice. A care coordinator can help identify participating practices that may accept your coverage, but the practice and insurer must verify network participation, claim authorization, benefits, and possible patient costs before care.

Your Care at a Crossroads
The future of medicine should move forward.
Without leaving the patient behind.
Concept illustration. Not actual TSCN clinicians or patients.
You should not have to choose.
Advanced technology. A doctor who knows you. Independent medical judgment. Personalized, collaborative care.
We believe you deserve all of them. The choice is not between progress and personal care. It is what we choose to make progress serve.
Speak with one of our doctors’ care coordinators. What is at stake
Progress is not the problem.
Losing the human relationship is.
Technology can help connect people, organize information, and support the work of medicine. But efficiency is not the same as care.
A patient is not a unit of production. A doctor is not simply a step in a workflow.
The purpose of progress should be to strengthen the relationship between them.
Who we stand beside
The doctors who choose to remain independent.
This is for the doctors who have chosen to remain in private practice—and the patients who seek their independent medical judgment.
It honors the responsibility those doctors accept: to listen, to think carefully, and to put the individual patient at the center of clinical decisions.
Independence should not have to mean working alone.
We can have both.
The tools of tomorrow. The personal commitment medicine has always required.
Judgment that remains with your doctor.
Clinical decisions remain with your treating doctor, informed by your needs, preferences, and circumstances.
Independent does not mean isolated.
Independent practices can work together, with care-coordination support to help organize referrals and permitted communication.
Technology in service of people.
Our vision includes carefully governed AI support to assist human work—not replace the doctor, the patient’s voice, or clinical accountability. This is a direction for development, not a claim that every envisioned capability is available today.
What TSCN stands for
Protect the independence.
Strengthen the connection.
We believe that in a world of purchased medical practices, we need TSCN to build a pathway to a better solution that supports the doctors and patients fighting to remain independent.
TSCN’s goal is to provide private-practice doctors and patients seeking independent, dedicated, personalized medical care with the most sophisticated technology platform on the planet. We are not there yet. With your support, we are determined to get there.
— Louis Cornacchia

Explore Patient MembershipBetter care happens when everyone works together.
From the hospital to the doctor’s office and throughout recovery, patients deserve a care team that communicates.
TSCN supports independent doctors with technology and care coordinators to help hospitals, health plans, and private practices work together toward timely, appropriate, and equitable care.

A stronger connection around every patient.
Our doctors bring hundreds of years of combined experience caring for patients with spine and joint conditions, pain, and injuries. We believe technology should support that experience at the point of care and help the whole care team stay connected.
Supporting hospitals and care transitions
Member physicians provide hospital care where they hold the appropriate privileges. TSCN’s coordination approach helps connect hospital care with the patient’s independent practice, rehabilitation, and follow-up.
Care coordinators help organize discharge information, records, appointments, and next steps so patients and their care teams have a clearer pathway after leaving the hospital.
Working with insurers for better care
Health plans have an important role in helping patients access care and keeping the healthcare system sustainable. We seek collaboration that improves communication between insurers and independent doctors.
Clearer clinical documentation, timely information, and technology that supports experienced doctors can help address coverage questions earlier. Our shared goal is to reduce avoidable costs while improving care quality and the patient experience.
Supporting doctors before, during, and after care
Every TSCN member doctor comes with care coordinator support. Coordinators help organize information before visits, support communication around the care plan, and assist with appointments and follow-up afterward.
Technology supports the doctor at the point of care. Clinical decisions remain with the treating physician, informed by the patient’s needs and preferences.
Fewer barriers. More equitable access.
We want to work with hospitals, insurers, and practices to prevent avoidable delays and denials of medically necessary care through clearer communication and complete documentation.
When disagreements arise, coordination can support clarification, clinical review, and appropriate appeals. Our aim is to help every patient access the most appropriate care for their individual needs, with dignity and respect.
Support that follows the patient.
Organize relevant records, referrals, appointments, and coverage questions.
Support the experienced clinician with useful information and a clearer picture of the patient’s needs.
Connect discharge plans, follow-up, rehabilitation, and communication with the ongoing care team.
Coordination without consolidation.
TSCN does not aggregate doctor offices. We believe doctors do their best work when they remain independent and in charge of their practices. Our technology and care coordinators strengthen collaboration while protecting the doctor’s clinical judgment and the patient’s choice.
