Tri-State Specialized Care Network

TSCN was created to help strengthen communication, coordination, and operational continuity among independent doctors and practices. Many doctors today face increasing fragmentation across referral pathways, administrative systems, scheduling processes, documentation flow, and patient navigation. TSCN is designed to support more organized collaboration while preserving the independence of participating practices.
Participating doctors remain fully independent and continue making all clinical, operational, and business decisions for their own practices. TSCN does not replace existing referral relationships, employment models, hospital affiliations, or payer relationships. Instead, the goal is to provide an additional coordination layer that may improve communication, continuity, and patient navigation across participating practices.
The long-term objective is to help independent medicine function more cohesively in an increasingly complex healthcare environment by improving coordination, responsiveness, operational efficiency, and professional connectivity among participating providers.
