Tri-State Specialized Care Network

TSCN (Tri-State Specialized Care Network) is a spine and joint care coordination and referral network connecting independent doctors across New York, New Jersey, and Connecticut. The network is designed to help patients move smoothly through the multiple sub-specialties that spine and joint care often require — and to help doctors collaborate with one another more directly than fragmented practice ordinarily allows.
TSCN is operated by TSCN, LLC (Limited Liability Company), which serves as the HIPAA (Health Insurance Portability and Accountability Act)-covered entity and the central administrative organization for the network. Member doctors participate through a Member Agreement and retain full ownership and clinical control of their own practices.
Apteum Spine
Apteum Spine is the patient-facing brand operated by TSCN, LLC. It is the consistent entry point through which patients reach the network — by phone, by website, or by referral. Apteum Spine handles intake, scheduling, and routing into the appropriate doctor’s practice, and it serves as the patient’s reliable point of contact across what is often a multi-step care journey.
Why TSCN exists
Spine and joint care is one of the most fragmented areas of medicine. A patient with a complex spine or joint condition may pass through pain management, interventional procedures, imaging, surgery, and rehabilitation — often across separate practices that communicate inconsistently. TSCN was built to organize that journey, by connecting the doctors involved through shared coordination, clear referral pathways, and a single patient-facing point of contact.
The result is intended to be straightforward: patients receive better-coordinated care, and the independent doctors who provide that care are able to collaborate without compromising the way they practice.
Founder and clinical leadership
TSCN was founded by Dr. Cornacchia, a neurosurgeon with more than 30 years of clinical experience specializing in robotic spine surgery, with a parallel background in medical device development and commercialization. The network’s design reflects that combined clinical and systems orientation.
