Tri-State Specialized Care Network

TSCN (Tri-State Specialized Care Network) is designed to simplify and modernize the intake and coordination process for patients. Rather than requiring patients to repeatedly print, scan, fax, or hand-deliver paperwork between offices, TSCN care coordinators (CC, Care Coordinators) can securely send electronic forms through a DocuSign-like process that patients can complete from their phone, tablet, or computer. Once submitted, the information is transmitted securely back to TSCN, where care coordinators review, process, and confirm receipt of the documents so the patient knows their information has been received and care can move forward without unnecessary delays or uncertainty. The goal is to reduce administrative frustration, improve communication, and allow patients to focus more on recovery and less on navigating paperwork.
