TSCN (Tri-State Specialized Care Network) is designed to make patient intake more organized, responsive, and less overwhelming during what is often a stressful and confusing period. The intake process frequently begins with an AI-assisted first contact that helps gather initial information such as symptoms, accident details, prior treatment history, current concerns, and immediate care needs. TSCN care coordinators (CC, Care Coordinators) then follow up to collect and verify additional demographic, scheduling, insurance, no-fault, or workers’ compensation information while helping guide the patient through the next steps of care coordination. Whenever appropriate, the patient’s primary care doctor can also play an important role by providing relevant medical history, medications, prior imaging, underlying health conditions, and continuity of care that may influence diagnosis and treatment decisions. After the administrative intake is completed, a more detailed clinical intake is performed by the appropriate participating doctor or clinical team — either in person or through telehealth — to evaluate the patient’s condition more thoroughly, review symptoms and imaging, assess function and goals, and help determine the most appropriate path forward for diagnosis, treatment, rehabilitation, and recovery. The overall goal is to reduce fragmentation, improve communication among doctors, and help patients move through care in a more connected and coordinated manner.
