Care is currently available through participating practices in New Jersey, New York City, and Long Island. If you live outside this service area, call 732-800-5548 before making travel plans to ask about available locations and whether a participating practice may fit your needs and insurance. TSCN is not an emergency service. If you believe you have a medical emergency, call 911 or seek care at the nearest emergency department.

Coordinated care can help clarify whether surgery is necessary.

TSCN is designed to help organize the right next step before you commit to an irreversible treatment.

Tired of being sent from doctor to doctor with no one talking to each other?

TSCN is a coordinated network of independent spine and joint specialists. One call, one care plan, one team — across physical therapy, chiropractic, pain management, and surgery.

TSCN doesn’t replace your doctors. We connect them.

We’re the concierge that ties your doctors to each other — and you to them. We sit between your specialists, making sure your care is coordinated, communicated, and on track.

Doctor to doctor

With appropriate patient authorization and secure processes, participating practices may coordinate relevant records and care information. Each practice remains responsible for its own documentation and clinical decisions.

Patient to provider

A TSCN care coordinator listens to your situation and helps identify participating doctors who may fit your needs, location, insurance, and goals. You choose the doctor you want to see.

Care that continues

When one participating provider hands off to another, TSCN coordination can support communication and help organize relevant care information for the next office. Each independent provider remains responsible for clinical decisions.

Why TSCN exists

Private practice doctors organized for resolution — not perpetual care.

TSCN doctors aren’t here to keep you coming back. We’re here to figure out what’s wrong, fix it where we can, and get you back to your life. Success means you don’t need us anymore.

Independent doctors, working together — coordinated by TSCN’s care coordinators and technology. Designed to be personal, connected, and centered on communication among independent doctors.

What we treat

Spine Surgery

For complex spine conditions that haven’t resolved with conservative care. TSCN spine surgeons emphasize restraint — surgery only when truly necessary, with minimally invasive and robotic options when it is.

Herniated disc · Sciatica · Spinal stenosis · Spondylolisthesis · Failed back surgery syndrome · Spinal instability · Compression fractures

Physical Therapy

Physical therapy may support recovery, rehabilitation, and management of some spine and joint conditions without medication. It may be considered as an early nonoperative option when appropriate after clinical evaluation.

Post-surgical rehabilitation · Sports injuries · Work injuries · Chronic back and neck pain · Mobility restoration · Strength rebuilding

Chiropractic

Manual care and alignment, integrated with the rest of your medical team rather than working in isolation.

Spinal alignment · Headache relief · Sports performance · Wellness maintenance · Adjunct care alongside PT or pain management

Pain Management

For chronic pain that needs more than physical therapy but doesn’t yet require surgery. Procedural and pharmacological approaches, carefully prescribed.

Spinal injections · Epidurals · Nerve blocks · Radiculopathy · Chronic pain syndromes · Post-surgical pain management

Joint Replacement Surgery

For knee, hip, and shoulder conditions that limit your daily life.

Knee replacement · Hip replacement · Shoulder reconstruction · Rotator cuff repair · Joint preservation procedures

How it works

1

Call

A care coordinator listens to your situation — your pain, your history, what you’ve tried.

2

Triage

An independent physician evaluates the patient and determines what care may be appropriate. Surgery is considered only when clinically indicated.

3

Meet

Patient Support may help connect you with a participating practice based on the general request and location, subject to practice fit and appointment availability.

4

Coordinate

Your providers communicate through TSCN throughout your treatment — and so do we, with you.

Even our coordinators have coordinators.

Jessie, TSCN’s AI receptionist, can receive questions and appointment requests at any time. A care-team member can review the request and follow up.

Insurance accepted

Participating independent practices may accept the following coverage, depending on the practice, location, claim, authorization, and insurer:

  • Workers’ Compensation — New York and New Jersey
  • No-Fault auto insurance (motor vehicle accident coverage)
  • Personal injury cases (liability and litigation)
  • Medicare

Participation and benefits must be confirmed for the selected independent practice. A care coordinator can help identify participating practices that may accept your coverage, but the practice and insurer must verify network participation, claim authorization, benefits, and possible patient costs before care.

Workers’ compensation patient? Call Patient Support at 732-800-5548 before traveling. If available, have your claim number, date of injury, employer, insurance carrier, adjuster contact information, injured body area, and any authorization details ready. TSCN cannot guarantee claim acceptance, authorization, appointment availability, or coverage.

Common questions

Does TSCN cost extra?

TSCN does not charge patients for calling Patient Support. Medical evaluation and treatment are billed by participating independent practices. Before care, ask the practice and your insurer or workers’ compensation carrier to confirm authorization, benefits, and any possible patient responsibility.

How is TSCN different from a regular referral?

A typical referral hands you a phone number and wishes you luck. TSCN follows the case. We share information across providers, track your progress, and help you navigate the next step. Think of us as your care navigator, not just a directory.

Will I always see the same doctor?

Not necessarily. Continuity depends on the specialty, participating practice, physician availability, location, insurance or claim authorization, and the patient’s choices. Ask the practice which clinician will provide care and how follow-up is handled.

What if I’m not sure what kind of care I need?

Call Patient Support at 732-800-5548. A care coordinator can listen to the general concern, organize the request, and help identify a potentially relevant participating practice. An independent clinician—not a care coordinator or AI—determines what evaluation or treatment may be appropriate.

What if surgery is the right answer?

We have experienced spine and joint surgeons in the network. We don’t push toward surgery unless it’s the right answer for your specific situation, and physicians — not AI — determine clinical care.

What insurance do you accept?

Participating independent practices may accept Workers’ Compensation in New York or New Jersey, No-Fault auto insurance, personal-injury matters, or Medicare, depending on the practice, location, claim, authorization, and insurer. Participation and benefits must be confirmed for the selected independent practice. A care coordinator can help identify participating practices that may accept your coverage, but the practice and insurer must verify network participation, claim authorization, benefits, and possible patient costs before care.

How long will I be in TSCN care?

As long as you need to be, and not a day longer. TSCN doctors aim for resolution — figuring out what’s wrong, fixing it, and discharging you back to your life. Our success metric is patients who don’t need us anymore.
